This page is pretty self-explanatory. Once you connect, the page will be enabled and the dropdown menus should also have the names of the Scripts in MECM and the Deployment Groups you have.

The dropdowns are filled in automatically from when you connected, but if they don’t fill in for some reason, make sure you are connected.

If you are correctly connected but the boxes didn’t fill in, just type in the name of the script and deployment group you want to run it on and it should work.
